Fuente Wellness Center is located inside of REACH Ashland Youth Center Youth and serve youth 11-24 years of age for FREE. Please register with Fuente and request an appointment for the services they desire. Fuente offers medical, dental, counseling and community health education.

Registration & Insurance Enrollment

Fuente Receptionist will register youth for Fuente services as well as schedule any needed appointments. Insurance enrollment by  appointment only.

Medical & Health Ed Appointments Mondays-Fridays 11:00pm-6:00pm        

Medical and Health Ed services include but are not limited to: sports physicals, regular physicals, TB testing, treatment of illness, Immunizations, Vision referrals, Asthma Management, Nutrition & Fitness Consultation, First Aid, Acne treatment, Confidential STI Screening/Prevention Counseling & Treatment, Pregnancy Testing/Options Counseling, Birth Control, Condoms.

 

Dental Services Mondays-Fridays 2:00pm – 6:00 pm

Oral screenings, Dental Cleaning, Dental Sealants, Fillings, Extractions (non-surgical only). Dental Hygiene Demonstration & Counseling, Referrals for Orthodonture/Braces, Wisdom Tooth Extraction (non-surgical only).
