REACH has a collaborative multi-agency Wellness team that supports the integration of health & wellness, social justice, and youth leadership into all of our programs and services.  Through our healing, capacity, and action framework, youth counselors participate in programming, build relationships, encourage skill building, and provide healthy mentorship with young people individually or in groups.  Wellness team members also provide wellness consultation to programs and partners and participate in our response and disciplinary interventions at REACH to bring a restorative, generative, and youth-focused lens to ensure young people are learning and growing from every challenge and success.

How To Make A Referrals

This referral form is used for: youth who may need support and guidance  with social-emotional development, basic life skills, goals or decision making; youth who have immediate needs or would benefit from coordination of care.

Anyone may refer a youth.


The referral form must be used for all Health and Wellness referrals (urgent or non-urgent) and for all probation/law enforcement/system involved referrals. Forms will be accepted in electronic and hard copy format.

Hard Copy Submission

Walk in referral submissions can be made at 16335 E. 14th St., San Leandro, CA 94578 at the Front Desk of the REACH Ashland Youth Center. We have a confidential locked box where this referral can be securely stored.

Mail/ Fax submissions

Fuente Wellness Center

Inside REACH, there is the Fuente Wellness Center, operated by La Clínica de la Raza, which provides primary medical services, individual and community health education services, dental services and behavioral health services through an integrated services model to youth ages 11-24. Health & Wellness services will include a mentorship/education/career pathway for youth exploring careers in the field. All programs and services will be offered at no cost to youth.

Registration & Insurance Enrollment

Mondays – Friday   10:00 am-6:00 pm
Fuente staff will register youth for Fuente services as well as schedule any needed appointments. Insurance enrollment appointments are available Mondays 10:00 am- 12:30 pm and Fridays 3:30 pm – 5:00 pm.

Medical & Health Ed Appointments

Medical and Health Ed services include but are not limited to: sports physicals, regular physicals, TB testing, treatment of illness, Immunizations, Vision referrals, Asthma Management, Nutrition & Fitness Consultation, First Aid, Acne treatment, Confidential STI Screening/Prevention Counseling & Treatment, Pregnancy Testing/Options Counseling, Birth Control, Condoms.

Dental Services

Mondays-Fridays   2:00pm – 6:00 pm
Oral screenings, Dental Cleaning, Dental Sealants, Fillings, Extractions (non-surgical only). Dental Hygiene Demonstration & Counseling, Referrals for Orthodonture/Braces, Wisdom Tooth Extraction (non-surgical only).
